Travel Grants for 2019 MTM-CNM Family Conference

MTM-CNM Family Connection, Inc is pleased to announce that through a generous 2019 RARE Patient Impact Grant from Global Genes, we are able to provide some financial assistance to families who attend the 2019 MTM-CNM Family Conference through Conference Travel Grants.
Eligibility & Parameters
A limited number of Conference travel grants are available to individuals and families affected by MTM or CNM who are participating in the 2019 MTM-CNM Family Conference. We hope that our conference travel grant program will provide support to families that may otherwise be unable to attend. While we cannot cover all the travel expenses for families, we can provide additional support to attendees who are in a situation where they need an extra boost in their travel budget in order to attend. The amount of each grant may vary based on need (family size, distance traveled), and total number of applicants, but will be on average $500.00.
All individuals affected by MTM/CNM and families are encouraged to apply!

Applications are being accepted as of now, until June 23rd, 2019. If the number of applications exceeds the available funding, priority will be given based on the following considerations:

Notification of grants: Sunday June30th, 2019
If awarded a grant, receipt submission deadline: August 5th, 2019
Grant checks mailed out: August 12th, 2019

- Hotel (during the conference)
- Other hotel if driving from greater than 5 hours away
- Gas (if driving to St. Louis)
- Airfare (if flying to St. Louis)
- Conference registration for family members/caregivers.
Applications are being accepted as of now, until June 23rd, 2019. If the number of applications exceeds the available funding, priority will be given based on the following considerations:
- Those families who will have a family member diagnosed with MTM or CNM in attendance at the conference
- Travel distance from St. Louis, MO
- First-time MTM-CNM Family Conference attendees
- Financial need

Conditions of Acceptance
If you are awarded a conference travel grant, you agree to the following conditions:
- Conference Registration: You must register for the conference prior to submitting the grant application, however registration fees are not due until the July 5th deadline and you will be notified prior to July 5th if you receive a travel grant. You may choose to apply funds from the grant directly toward your registration fees if desired, once awarded
- Full Attendance: We request you attend the full duration of the conference program (Friday afternoon through Sunday) when applying for the travel grant.
- Use: Use grant funds explicitly for conference-related expenses.
- Sharing your experience: By accepting the travel grant, you agree to complete a post-event survey and provide written reflections, photos, and/or video on your conference experience to MTM-CNM Family Connection by August 5th, 2019. Your feedback may be shared by MTM-CNM Family Connection for purposes related to furthering our mission and promoting future activities, and specifically with Global Genes for the RARE Impact Award reporting and dissemination of patient impact.
